About Harmony Support Service
Harmony Support Service is a self-directed community support service that operates in the Gold Coast region. The focus of this service is to provide high quality service within a recovery framework. It aims to facilitate the development of independence within the community and assists individuals to build their personal, natural and community resources. Individuals accessing this service are assisted to develop their own individual recovery plan and to identify the areas within their lives that they need support. They are encouraged to maintain their roles and responsibilities in order to continue to live independently, develop their self mastery and reclaim their citizenship within their communities.
How Harmony Support Service can help you
Harmony Support Service is supported by dedicated Fellowship employees who are trained using a strengths-based recovery framework.
Individuals accessing this service are assisted with developing a recovery plan using a goal orientated framework.
Goals included in this plan, may include shopping independently, managing a budget or accessing other mental health or community services.
Referrals to Harmony Support Service
To access the program you must be over 18 years old and diagnosed with a psychiatric disability. Referrals can come from medical professionals, government departments, as well as family and friends.
Just follow the links to download and print the referral form and consent form to apply for Harmony Support Services. Both documents must be returned to process the referral.
There is no cost.
How Harmony Support Service is funded
Harmony supports individuals through block funds and individual support packages, including Adult Lifestyle Support Program (ALSP), Project 300 (P300), Housing and Support Program (HASP) and Hostel Response.
Harmony Support Service is funded by the Department of Communities and currently supports approximately 30 to 40 individuals.
