Individualised Support for Community Living

Both Aftercare and MIFWA are working in partnership to deliver the Individualised Community Living Strategy (ICLS) or otherwise referred to as the ‘Individualised Support for Community Living’ program.

Participants of ICLS are either long stay hospital patients or living in the community.   The program assists individuals to access accommodation if required and to live independently in the community.

The program uses a person centered approach, by identifying individual needs, and supporting people to live independently in the community. The individual consumers are allocated to this service based on various factors including the consumer’s choice, previous connection with the service providers, location and approach of the service providers and the support and services we are able to deliver. The services provided in this program range from low to high care individual support packages depending on the individual need.

Aftercare has recently located to Perth and currently delivers mental health and disability services that support people with mental illness in New South Wales and Queensland, including high level accommodation support, outreach programs, Headspace youth services and the Personal Helpers and Mentors Program. The service is funded for the North Metropolitan region.

Referrals for Housing and Support for ICLS Consumers are only accepted from the Mental Health Commission.
